Program Assistant/Designated Manager

A Program Assistant  is someone who works to empower and support the individuals we serve to achieve their goals and dreams. They are specifically and competently trained to work with the individual in their home and are confidently and actively engaged as an effective and cooperative member of the person’s support team.  The Program Assistant implements the person’s service plan and encourages and supports them to make choices, decisions and plans that reflect their personal interests. A Program Assistant is committed to providing services in a manner that reflects Creative Living Group’s mission, vision, and values. A Program Assistant is a full-time position with 40 hours per week with varying days and shifts depending on the needs of the program.  If there is an open position, the Program Assistant may work those hours until a new employee is hired for that position.  If a staff member is unable to work their shift, the Program Assistant will be the first line of defense either to cover that shift or to cover someone else’s shift so they can cover the open shift.  The Program Assistant’s schedule will be determined each week by the Program Manager.

Your Responsibilities

  • Provide direct and personal care, and support activities of daily living with the person receiving services. Prioritize and accomplish work responsibilities related to direct care in a timely manner using good organizational skills and attention to detail.

  • Ability to remain present and observant of the individual served.

  • Support the person with morning and/or bedtime routines, showers, grooming, oral care, foot care, and peri care.

  • Become knowledgeable of the person’s diet requirements and prepare attractive, healthy meals within those guidelines. Eat nutritious meals with the person respecting their limitations.

  • Be aware of medical needs and report to the guardians and Program Manager any changes. Ability to react quickly in crisis situations.

  • Provide positive examples and appropriate support regarding interpersonal behavior.

  • Plan, participate, lead community outings as requested by the individual and included in their service plan.

  • Creates new opportunities for individuals to increase new social relationships.

  • Maintain an environment that is safe, clean, comfortable and supportive of the person receiving services. Report items that need repair, complete cleaning list responsibilities, and communicate supply and equipment needs in order to successfully complete cleaning and other duties.

  • Communicate respectfully and effectively with person receiving services, guardians, Program Manager, team members, and coworkers. Update/read communication book, have verbal discussions during shift changes, read and respond to emails, voicemails, and other requests for information timely – see Management Communication guidelines.

  • Provide quality and accurate documentation as needed, including incident reports, daily notes, medication administration, data sheets, day program books, and all checklists. Print and update them when needed and upon request. Manage the person’s required financial reporting, tracking petty cash, and receipts as requested.

  • Provide transportation with CLG provided vehicle and successfully support the person on medical appointments as scheduled in a professional manner and follow up with documentation, ordering/transcribing medications and supplements.

  • Dress professionally and appropriately including following the dress code for the person’s place of work.

  • Personal use of your cell phone is restricted to emergency calls and work communication during your shift.

  • Maintain good relationships and support with your coworkers. Refrain from any gossip and negative conversations and dissuade your coworkers from doing so.

  • Respect your coworkers by coming to your shift on time. You are responsible to stay until the next staff person arrives. You are responsible for covering your shifts if you are unable to work your shift. Communicate with your Program Manager or if not available, Director.

  • Finances are recorded daily with receipts, checked each week and reconciled at month’s end.

  • Assist with books and processes – create and maintain activity calendar, community book, recipe book, menu for each week, program book, medical book, communication book, etc. Make grocery and supply lists each week or when needed.

  • Assist in creating opportunities for social interactions for person served and staff – help organize community outings and get togethers, birthday parties, other special occasions, staff recognition and team building ideas.

  • Assist in creating the MAR each month and when updated with new medications/supplements. On your shifts, check for accuracy and signatures are complete.

  • Assist in setting up the medicine trays for the week. Have another staff be the 2nd check.

  • Cleaning – participate in making sure the house and laundry is clean. Assist individuals in increasing their housekeeping skills at their ability level. Maintain a clean refrigerator and staff’s refrigerator. Ensure that staff clean up after themselves including washing off the stove top, microwave, counters, dishes, etc. Clean bathroom and other areas per staff cleaning schedule.

  • On-call responsibilities to ensure day and overnight shift coverage.

  • Ability to handle staff issues on phone and in person, de-escalate and problem solve situations and discuss appropriate solutions together with those involved.

  • Assist in creating a team with the staff – help staff support one another, become well trained and confident, and create a healthy, open and fun atmosphere.

  • Ability to work closely with and have a good relationship with guardians.

  • Assist Program Manager in providing feedback for performance reviews and corrective actions.

  • Complete required trainings as scheduled, attend mandatory staff meetings, and maintain 245D requirements.

  • Complete other tasks and responsibilities as assigned and/or needed to ensure the agency meets its mission.
